CONVERSION SPECIFICATIONS AND PRICE LIST

1. Full File Conversion Cost: Php 150,000.00
(Price includes up to 10,000 patients. Additional patients are Php 10.00 each)

A Full File data conversion provides history information as well as patient demographics and insurance data. All other features of a Standard Conversion are also included. This option is unique in that it provides the client with critical medical records going forward in the new system. A Full File Conversion includes the following data categories, subject to available data:

• Account Balances
• Financial Transactions
• Patient Information
• Employer Information
• Dependent Information
• Name Parsing
• Referring Provider
• Insurance Carrier Information
• Procedure Codes
• Diagnosis Codes
• Address Book Information
• Appointment/Scheduling Information

2. Standard Conversion Cost: Php 100,000.00
(Price includes up to 10,000 patients. Additional patients are Php 7.00 each)

We convert basic Patient information to give your client a populated ComLogik database. The converted data will be as accurate and complete as the data in the "old system". An automated data conversion is considerably more accurate than a manual conversion and can usually be done over a much shorter period of time. In addition, our conversion software will clear up and standardize common data problems like street nomenclature, abbreviations, directional designations, first name/last name sequence etc. A Standard conversion includes the following data categories, subject to available data:

• Employer Information
• Name Parsing
• Insurance Carrier Information
• Procedure Codes
• Diagnosis Codes

3. Economy Conversion Cost: Php 80,000.00
(Price includes up to 7500 patients. Additional patients are Php 5.00 each)

For clients who are particularly cost sensitive but who still see value in an electronic conversion, this is a great choice. This option also allows for a shorter turn-around time on the conversion. An Economy Conversion includes the following data categories, subject to available data:

• Basic Patient Information
• Procedure Codes
• Diagnosis Codes


4. Hard Copy Conversion Cost: Php 120,000.00
(Price includes up to 5000 patients. Additional patients are Php 5.00 each)

Converting data from hard copies (paper) will have to be scanned and reproduced as JPEG format to be link-up with the HIMS to be accessible (every time they pull data).

Turn Around Time and Scheduling
Because our staff is dedicated to data conversions we can provide the shortest turn around time practical. Usually this means one to two weeks. It can be done in less time if we have sufficient notice to coordinate our scheduling with you and the Client.
